Tooth Enamel Erosion: Causes and Ways to Protect Your Teeth

Tooth enamel erosion is the gradual dissolving of the tooth's outer protective layer (the enamel) under the effect of acids. The main causes are fizzy and acidic drinks, acidic foods, stomach acid coming back up into the mouth (reflux or vomiting) and dry mouth. Unlike tooth decay, which is caused by bacteria, erosion is a chemical process. Once enamel is lost it does not grow back naturally, which is why prevention matters even more than treatment.
What does the research show? The ORCA/IADR consensus (2020) defines the term “erosive tooth wear” as the combined effect of bacteria-free chemical dissolution (erosion) and mechanical wear (brushing, chewing, clenching) — that is, acid softens the enamel and mechanical forces then remove the softened layer. The acid source may be external (fizzy and acidic drinks, citrus) or internal (reflux, vomiting); in adolescents, the link between fizzy drink consumption and erosion has been confirmed by meta-analysis. Dentists record the degree of wear using the BEWE index; this is a different process from decay, and its management consists of reducing acid contact and protecting the teeth with fluoride/stannous-containing products — deeper defects may require restoration. What is tooth enamel erosion?
Enamel is the hardest tissue in the body and protects the tooth from everyday forces. During erosion, acids dissolve the mineral content of the enamel and the layer becomes thinner. This is different from tooth decay: decay is caused by the acid produced by bacteria, whereas erosion is the direct effect of food, drink or stomach acid. In both cases the result is similar — the tooth becomes weaker.
Main causes
The leading causes of tooth enamel erosion are:
- Acidic drinks — fizzy drinks, energy drinks, fruit juices and sour lemonade.
- Acidic foods — citrus fruits, vinegar-based foods and some sour snacks when eaten frequently.
- Stomach acid — gastro-oesophageal reflux (GERD) or frequent vomiting can wear the enamel away from the back surfaces.
- Dry mouth (xerostomia) — saliva neutralises acid; when it decreases, this protection weakens.
- Incorrect brushing habits — brushing hard immediately after acidic food or drink can abrade the softened enamel.
Early signs
Erosion develops slowly and may not be noticeable at first. Signs to watch for:
- Sensitivity — increased sensitivity to hot, cold or sweet.
- Colour change — teeth appearing more yellow (the underlying layer showing through).
- A shiny or smoothed surface — thinning of the enamel.
- Translucent edges — the biting edges of the front teeth taking on a glassy appearance.
With these signs, tooth sensitivity is often the first warning signal.
Ways to protect your teeth
Erosion can be prevented:
- Limit acidic drinks: Cut down on fizzy drinks; when you do have them, use a straw so the liquid has less contact with your teeth.
- Do not brush straight away: After acidic food or drink, wait at least 30–60 minutes — the enamel is soft, so brushing immediately causes harm. In the meantime, rinse your mouth with water.
- Drink water: Keeping the mouth moist and washing acid away supports the work of your saliva.
- Fluoride toothpaste: Helps to strengthen the enamel.
- Manage reflux: If you often feel stomach acid, discuss it with your family doctor — this protects both your teeth and your general health.
- Regular check-ups: Professional cleaning and check-ups catch the early signs.
When should you see a dentist?
- Ongoing or increasing tooth sensitivity
- A change in the colour or shape of your teeth
- Frequent stomach acid or a feeling of reflux
- Shortening or wear of the tooth around old fillings
